Job Summary:
Provide specialized physical therapy services to patients in a home health setting, delivering care that promotes recovery and improves quality of life. Ensure treatment plans are effectively implemented while maintaining compliance with industry standards.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Administer therapeutic exercises and manual therapy techniques.
• Document patient progress and update care plans accordingly.
• Communicate effectively with patients, family members, and healthcare teams.
• Maintain accurate and timely records of all clinical activities.
• Comply with all agency policies, licensing requirements, and regulatory standards.

Qualifications:

• Hold an active CA license as a Physical Therapist.
• Possess a minimum of 1 year of home health or outpatient experience.
• Demonstrate proficiency with Oasis documentation and requirements.
• Have current BLS (AHA) certification.
• Be able to operate personal transportation and have a valid driver’s license.
• Maintain strong communication and organizational skills.
