Job Summary:
Seeking a dedicated Physical Therapist for a field position in home health. The role involves delivering patient-centered care in a flexible schedule, with opportunities for local or travel assignments based on experience and licensure.

Responsibilities:

• Evaluate patient needs and develop personalized treatment plans for home health clients.
• Administer therapy services and monitor patient progress to ensure optimal recovery.
• Document patient care and clinical notes accurately in compliance with agency standards.
• Communicate promptly with healthcare teams regarding patient status and treatment updates.
• Participate in on-call rotations as required to provide after-hours support.
• Travel between patient locations efficiently, utilizing personal transportation when necessary.

Qualifications:

• Hold an active California license in good standing.
• Possess a minimum of 1 year of Home Health experience or outpatient experience if no Home Health background.
• Demonstrate experience with Oasis documentation; candidates without Oasis may be considered with strong Home Health experience.
• Maintain BLS certification through AHA.
• Have a valid driver’s license and reliable transportation.
