Job Summary:

The Physical Therapist provides specialized rehabilitation services to residents in a skilled nursing setting, aiming to maximize functional abilities and promote independence through customized therapy plans. They collaborate with the healthcare team to deliver resident-centered care and ensure optimal recovery outcomes.

Responsibilities:

• Perform evaluative and therapeutic procedures in accordance with physicians’ orders and regulatory guidelines.
• Develop and implement individualized treatment plans with measurable goals, adjusting interventions based on resident progress.
• Educate residents and caregivers on therapeutic exercise, assistive device use, and safety to support ongoing recovery and independence.
• Document all evaluations, treatments, and educational interactions accurately in the Electronic Medical Record, ensuring clarity and compliance with reimbursement requirements.
• Coordinate care with physicians, therapy assistants, and other healthcare team members to optimize therapeutic outcomes and facilitate smooth discharge planning.
• Supervise and mentor assistant-level staff, providing guidance, ongoing training, and performance feedback to ensure quality standards are met.

Qualifications:

• Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferred.
• Active licensure as a Physical Therapist in the applicable state.
• Prior experience in long-term care or skilled nursing facilities is preferred.
• Maintain current Basic Life Support (BLS) certification and professional licensure renewal.
• Strong communication, teamwork, and clinical decision-making skills.
