Job Summary:
The Physical Therapist will provide patient-centered therapy services in a home health setting, supporting recovery and mobility improvement for diverse patient populations. This role involves traveling between patients' homes, adhering to clinical standards and documentation protocols.

Responsibilities:

• Evaluate patient needs and develop personalized treatment plans.
• Deliver therapy sessions, demonstrating techniques and ensuring patient safety.
• Document progress and update medical records accurately.
• Collaborate with care teams to coordinate ongoing patient care.
• Adhere to travel compliance requirements, including licensing and certifications.
• Ensure visit completion for hours guarantee and maintain documentation standards.

Qualifications:

• Hold a valid California PT license or be pending licensure application.
• Demonstrate experience with Oasis documentation preferred; home health experience is a plus.
• Possess BLS/CPR certification.
• Be willing to travel between patient locations and complete a minimum of 25 visits for guaranteed hours.
• Maintain compliance with travel regulations and standards.
