Job Details

Job Summary:
Provide skilled nursing care to residents in long-term care or skilled nursing facilities. Administer treatments, monitor patient conditions, and support the healthcare team in delivering quality patient care.

Responsibilities:

• Deliver direct nursing care to residents in accordance with the care plan.
• Monitor and document patient conditions, including vital signs and medication administration.
• Collaborate with interdisciplinary teams to develop and update care strategies.
• Comply with all facility policies, including identification and badge requirements, and adhere to safety protocols.
• Maintain accurate and timely documentation of care provided.
• Follow infection control procedures, including COVID-19 vaccination and exemption documentation.

Qualifications:

• Valid and current LVN or LPN license.
• Proven experience working in LTC or SNF settings preferred.
• Ability to adhere to facility policies regarding identification and vaccination status.
• Strong patient care and communication skills.
• Demonstrated proficiency in documentation and clinical procedures.
• Must bring a valid photo ID and meet vaccination or exemption requirements.
