Job Summary:
Seeking a dedicated RN in Case Management to support patient coordination and discharge planning at a leading regional hospital. This role involves collaborative patient care, documentation accuracy, and effective communication within a fast-paced healthcare environment.

Responsibilities:

• Coordinate patient care plans in collaboration with multidisciplinary teams.
• Document patient interactions and care activities accurately using CERNER, NATE, and ENSOCARE systems.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
• Manage shifting attention between tasks while maintaining bedside trust and professionalism.
• Support hospital policies, unit guidelines, and safety protocols.
• Adapt to floating policies and assist where needed across units.

Qualifications:

• Licensure as a registered nurse with a current NC state license.
• Minimum two years of nursing experience in hospital case management.
• Demonstrated ability to communicate clearly and build trust with patients and colleagues.
• Proven documentation skills using electronic health record systems.
• Ability to work self-scheduled shifts and adapt to unit needs.
• Strong teamwork and collaborative skills.
