Job Summary:


Provide compassionate, comprehensive hospice nursing care to patients in a home-like setting, supporting their physical, emotional, social, and spiritual needs during end-of-life stages. This role emphasizes teaching, assessment, and symptom management to enhance patient comfort and caregiver confidence.



Responsibilities:

• Deliver personalized end-of-life care focused on patient comfort and dignity.
  
• Educate patients and caregivers on care plans, symptom management, and goal setting.
  
• Assess patient conditions regularly to identify immediate needs and adjust treatments accordingly.
  
• Collaborate with the interdisciplinary team to develop and update individualized care plans.
  
• Maintain accurate documentation of patient status, interventions, and care outcomes.
  
• Participate in a 3-day orientation and provide on-call support, averaging 2 shifts per month.
  
  
  
  Qualifications:
  
• Hold a valid Registered Nurse (RN) license in the appropriate state.
  
• Demonstrate strong clinical skills in symptom management and patient assessment.
  
• Possess experience or familiarity with hospice or palliative care preferred.
  
• Exhibit excellent communication and teaching abilities for patient and family support.
  
• Show ability to work independently and as part of a multidisciplinary team.
  
• Capable of providing compassionate and culturally sensitive care.
