Job Summary:

Provide comprehensive nursing care to patients in a long-term care or skilled nursing facility setting. Deliver quality patient assessments, treatments, and support to promote optimal health outcomes.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document patients’ vital signs, symptoms, and overall condition.
• Educate patients and their families about health management and discharge instructions.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Maintain compliance with facility policies, state, and federal regulations.
• Ensure all documentation is thorough, accurate, and completed in a timely manner.

Qualifications:

• Valid Registered Nurse (RN) license in the relevant state.
• Minimum of 1-2 years of experience in LTC or SNF settings.
• Current CPR and BLS certification.
• Ability to work a rotating schedule and adapt to changing patient needs.
• Strong clinical assessment and documentation skills.
• Proven ability to follow infection control protocols and safety procedures.
