Job Summary:
Provides Skilled Nursing care to residents in a licensed healthcare facility under the supervision of a Registered Nurse and Director of Nursing. Administers medications, assesses resident conditions, and collaborates with the healthcare team to ensure quality care.

Responsibilities:

• Assess and reevaluate residents’ conditions to develop and implement appropriate nursing care plans.
• Administer medications following the eight (8) medication rights to minimize errors.
• Monitor and interpret results of waived point-of-care tests, taking necessary actions accordingly.
• Document resident care, changes in condition, and physician orders accurately and timely.
• Communicate effectively with residents, families, and team members regarding care needs and updates.
• Supervise CNAs and support staff to ensure proper care delivery and compliance with policies.

Qualifications:

• Graduate of an accredited LVN/LPN program.
• Current California LVN/LPN license.
• Valid BLS certification.
• Ability to read, write, and communicate effectively in English.
• Basic computer skills; experience with Electronic Health Records preferred.
• Demonstrate competence in clinical skills and adherence to infection control protocols.
