Job Summary:
The LVN / LPN – LTC Float Pool position involves providing high-quality nursing care across multiple units, including LTAC, LTC, Behavioral Health, and medical-surgical settings. The role requires flexibility to support various patient populations while maintaining compliance with facility policies.

Responsibilities:

• Assess patient needs and deliver personalized nursing care in diverse clinical environments.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Support both medical and Behavioral Health patients, utilizing behavior-management techniques.
• Float between units such as LTAC, skilled nursing, Medical-Surgical/Telemetry, and Behavioral Health as needed.
• Complete orientation and ongoing training, including virtual modules, to stay current with facility protocols.
• Adhere strictly to attendance policies, including punctuality and holiday shift requirements.

Qualifications:

• Valid Maryland or Compact LVN / LPN license required at submission.
• Minimum of 2 years of LVN / LPN experience, LTAC experience preferred.
• BLS certification is mandatory.
• Experience caring for patients with degenerative disorders, schizophrenia-spectrum, or behavioral health needs preferred.
• Comfortable supporting both medical and behavioral health units and floating within scope.
• Knowledge of Cerner and PointClickCare systems is a plus.
