Job Summary:
The Registered Nurse provides skilled nursing care within a Long-Term Care/SNF setting, ensuring patient safety and comfort through comprehensive assessments and interventions. This role involves collaborating with healthcare teams to deliver high-quality, patient-centered care to elderly residents.

Responsibilities:

• Administer medications and treatments following physician's orders and facility protocols.
• Conduct patient assessments and develop individualized care plans.
• Monitor patient progress and document clinical findings accurately.
• Collaborate with interdisciplinary teams to coordinate care and implement individualized interventions.
• Ensure compliance with all health and safety regulations, including infection control procedures.
• Educate residents and their families on health management and care plans.

Qualifications:

• Current Registered Nurse license in the state of practice.
• Proven experience in LTC/SNF or related geriatric care settings.
• Ability to perform detailed assessments and develop care plans.
• Strong communication and teamwork skills.
• Knowledge of infection control practices and safety protocols.
• BLS certification required; additional certifications preferred.
