Job Summary:
Provide expert case management nursing services in a hospital setting, coordinating patient discharges and ensuring appropriate care plans. Collaborate with interdisciplinary teams to optimize patient outcomes.

Responsibilities:

• Assess patient needs and develop comprehensive discharge plans.
• Utilize InterQual criteria to verify patient medical status.
• Communicate effectively with healthcare providers, patients, and families.
• Document case management activities in the electronic medical record system.
• Float to various med-surge or ICU units as needed.
• Adhere to facility policies and maintain professional standards.

Qualifications:

• Hold a valid RN license for practice.
• Possess at least 2 years of hospital case management experience.
• Demonstrate flexibility and strong communication skills.
• Capable of working in fast-paced, dynamic environments.
• Comfortable asking questions and adapting to unit policies.
• Experience with Meditech/Meditech Expanse EMR preferred.
