Job Summary:
Provide outpatient physical therapy services to patients in the NW Philadelphia and Montgomery County areas. Assess, plan, and implement treatment programs to improve patient mobility and function.

Responsibilities:

• Evaluate patients’ physical conditions and develop individualized treatment plans.
• Administer therapy sessions focusing on improving strength, flexibility, and mobility.
• Document patient progress and update care plans in EPIC and OASIS systems.
• Perform start of care assessments and follow-up visits as needed.
• Collaborate with healthcare teams to ensure coordinated patient care.
• Maintain accurate and timely documentation for all patient interactions.

Qualifications:

• Hold a valid and current license as a Physical Therapist.
• Demonstrate recent home health experience.
• Proficient in EPIC electronic medical record system.
• Familiar with OASIS documentation requirements.
• Have prior experience with start of care and revisit assessments.
• Possess strong clinical assessment and communication skills.
