Job Summary:
The Physical Therapy Assistant (PTA) will provide home health therapy services, assisting patients with their rehabilitation goals under the supervision of a licensed therapist. This role involves home visits, documentation, and coordinating patient care within assigned territories.

Responsibilities:

• Deliver skilled physical therapy services to patients in their homes.
• Assist in developing and implementing treatment plans based on therapy guidelines.
• Document patient progress and communicate updates to supervising therapists.
• Travel to multiple patient locations within designated regions, maintaining timely appointments.
• Follow agency protocols for documentation, patient safety, and infection control.
• Participate in periodic office meetings and training sessions as required.

Qualifications:

• Hold an active and valid state license as a Physical Therapy Assistant.
• Minimum of 1 year of recent adult home health therapy experience.
• Possess current CPR and BLS certifications.
• Have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Able to travel up to 75 miles from home clinic and manage a caseload of approximately 5-6 patients daily.
