Job Summary:
Seeking an experienced SW Social Worker for a travel assignment in a Level I Trauma facility. The role involves providing psychosocial support, discharge planning, and assessment services in a fast-paced acute care environment.

Responsibilities:

• Evaluate patients' psychosocial needs and develop individualized care plans.
• Perform comprehensive psychosocial assessments and document findings accurately.
• Collaborate with interdisciplinary teams to ensure seamless discharge planning.
• Utilize Cerner EMR and Care Port platforms to record clinical information.
• Educate patients and families on advanced directives and social determinants of health.
• Maintain high standards of customer service while managing urgent care situations.

Qualifications:

• Must have at least 2 years of experience in a Level I Trauma hospital setting.
• Hold a valid Florida or Compact nursing license at the time of submission.
• Experience working with Cerner EMR and Care Port systems.
• Demonstrate proficiency in discharge planning and psychosocial assessments.
• Must possess a Bachelor’s degree in Social Work or related field.
• Prior travel experience in similar settings is required.
