Job Summary:
Provide skilled physical therapy services to patients in a home health setting, focusing on improving mobility, strength, and overall wellness. Collaborate with patients and caregivers to develop personalized treatment plans and ensure quality care.

Responsibilities:

• Assess patients’ physical conditions and develop treatment plans tailored to their needs.
• Implement therapeutic exercises and interventions to improve mobility and function.
• Educate patients and caregivers on exercises, mobility techniques, and injury prevention.
• Monitor, document, and report patient progress and treatment outcomes accurately.
• Adjust therapy plans based on patient response and evolving needs.

Qualifications:

• Current state licensure as a Physical Therapist.
• Valid CPR certification.
• Proven experience in home health or outpatient physical therapy preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
