Job Summary:
Seeking a licensed Physical Therapist to provide home health services, including initial assessments, follow-ups, and rehabilitation for adult patients. The clinician will deliver care through home visits, ensuring quality and compliance with agency policies.

Responsibilities:

• Evaluate patients’ physical conditions and develop individualized treatment plans.
• Perform therapeutic interventions and document patient progress accurately.
• Travel to patient homes within a 75-mile radius, providing timely care.
• Complete all documentation within 24 hours of each patient encounter.
• Adhere to agency protocols, including on-call rotations and compliance requirements.
• Collaborate with interdisciplinary teams during periodic meetings and coordinate care as needed.

Qualifications:

• Hold a current state license as a Physical Therapist.
• Minimum of 1 year of recent adult home health experience.
• Proficient in HomeCare HomeBase EMR and OASIS documentation.
• Must have reliable transportation, driver’s license, and auto insurance.
• Comfortable with travel up to 75 miles and accepting a full patient schedule.
• Ability to work independently and manage a full caseload efficiently.
