Job Summary:
Provide Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) care in a long-term care or skilled nursing facility setting, ensuring quality patient care and compliance with healthcare standards.

Responsibilities:

• Administer medications and treatments according to physician orders.
• Monitor and document patient conditions regularly.
• Assist with activities of daily living and patient comfort.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Maintain accurate and complete documentation of patient records.

Qualifications:

• Hold a valid LVN or LPN license in the state.
• Demonstrate at least one year of experience in LTC or SNF settings.
• Possess current CPR and basic life support (BLS) certifications.
• Be able to effectively communicate with team members and patients.
• Comply with health and safety regulations, including COVID-19 vaccination requirements or valid exemptions.
