Job Description
Job Summary:
Seeking a highly experienced RN Case Manager with inpatient acute care expertise to coordinate care transitions and optimize resource utilization. This role involves collaborating with interdisciplinary teams to ensure seamless patient journeys through the hospital stay and beyond.
Responsibilities:
- Conduct comprehensive patient assessments upon admission and throughout hospitalization to identify risks and care needs.
- Develop and implement individualized, cost-effective care plans in coordination with physicians, social workers, and other healthcare professionals.
- Evaluate ongoing patient status to ensure appropriate level of care and facilitate timely transitions to lower levels or discharge planning.
- Lead and supervise care management staff and assist in addressing complex clinical and social issues to prevent unnecessary readmissions.
- Ensure compliance with regulatory standards such as CMS and The Joint Commission, and document all activities accurately in the electronic health record.
- Collaborate with payers, community resources, and families to address barriers to care and coordinate post-discharge services.
Qualifications:
- Current California RN license with an active Nursys verification uploaded within the last 2 weeks.
- ?5 years of RN experience, specifically in hospital-based case management and inpatient acute care settings.
- Valid BLS certification, with additional relevant credentials highly preferred.
- Proven ability to lead interdisciplinary teams and manage complex patient cases effectively.
- Strong knowledge of CMS, Conditions of Participation, and regulatory compliance in transitional care.
- Excellent communication, organizational, and problem-solving skills.
