Job Summary:

The Social Worker provides essential support services to patients and their families, assisting them in adapting to illness, injury, or hospitalization. They collaborate with healthcare teams to address social, emotional, and economic factors impacting patient recovery and well-being.

Responsibilities:

• Evaluate patients' social and emotional needs through comprehensive assessments.
• Facilitate timely discharge planning, coordinating with various community and healthcare facilities.
• Identify barriers to discharge and develop strategies to overcome them.
• Intervene in cases involving child abuse, neglect, domestic violence, guardianship, foster care, mental health, and adult/elderly abuse.
• Access and mobilize community resources to support patients' ongoing needs.
• Educate patients and families on palliative care options and end-of-life decisions.

Qualifications:

• Minimum of 2 years hospital social work experience required.
• Proven experience in discharge planning required.
• Experience working across all patient populations required.
• Valid State of Minnesota Social Work License (LSW) required.
• Proficient in Epic EMR system required.
• Must provide current skills checklist and supervisory references.
