Job Summary:
Seeking an experienced RN - Case Management for a 12-week travel assignment at a reputable medical center. This role involves coordinating patient discharge plans and managing observations to ensure optimal patient outcomes.

Responsibilities:

• Assess patient needs and develop personalized care and discharge plans.
• Collaborate with medical teams, social workers, and patients to facilitate smooth transitions of care.
• Utilize documentation systems such as Midas, Meditech, and Well Sky for accurate record-keeping.
• Prioritize patient safety and compliance with unit policies and guidelines.
• Participate in team meetings and contribute to department goals focused on patient experience and quality.
• Maintain certifications and adhere to hospital dress code and policies.

Qualifications:

• Current RN license with valid state registration.
• Minimum of 1 year of experience in case management or a related clinical setting.
• Proficiency in discharge planning, observation management, and InterQual criteria.
• Strong communication and interdisciplinary collaboration skills.
• Ability to manage multiple priorities within a fast-paced hospital environment.
• Knowledge of hospital documentation systems such as Midas, Meditech, or Well Sky.
