Job Summary:
Provide comprehensive nursing care to patients in home health and hospice settings. Deliver personalized clinical support while ensuring compliance with regulatory standards and patient needs.

Responsibilities:

• Administer skilled nursing care to patients in home health and hospice environments.
• Perform comprehensive assessments and develop individualized care plans.
• Document OASIS assessments accurately and maintain detailed patient records.
• Collaborate with interdisciplinary teams to coordinate patient care and ensure optimal outcomes.
• Educate patients and family members on health management and disease prevention.
• Ensure adherence to safety standards and uphold professional nursing practices.

Qualifications:

• Valid RN license with current registration.
• Minimum of one year of experience in home health and hospice care.
• Proven experience with OASIS data collection and documentation.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to wear Navy or royal blue scrubs comfortably; team members commonly wear WVU t-shirts.
