Job Summary:
Seeking experienced LVN/LPN professionals to provide outpatient dialysis care in a high-volume outpatient setting. The role involves delivering specialized renal treatment to chronic and acute patients, ensuring safety and compliance with all protocols.

Responsibilities:

• Administer dialysis treatments to outpatients with chronic and acute conditions.
• Monitor and interpret key alarms and system parameters such as dialysate conductivity, pressure levels, air foam detector, and temperature alarms.
• Cannulate AV fistulas and grafts, and manage tunneled or non-tunneled catheters while maintaining infection control standards.
• Document patient assessments and treatment data accurately using EPIC or similar electronic charting systems.
• Respond promptly to emergencies such as blood leaks, clotting, hypotension, or cardiac events, including CPR when necessary.
• Collaborate with multidisciplinary teams to ensure patient safety and optimal outcomes during treatment sessions.

Qualifications:

• Valid NY LVN/LPN license required in hand prior to start.
• Minimum of 1 year of outpatient dialysis experience in chronic or acute settings.
• Proven ability to cannulate AV fistulas and grafts safely and effectively.
• Strong knowledge of dialysis equipment, alarms, and troubleshooting procedures.
• Experience with EPIC or equivalent electronic health records system.
• Ability to work flexible shifts, including day/evening hours, with rotating weekends.
