Job Summary:
Seeking an experienced travel Physical Therapist to provide skilled therapy services in a busy rehab unit. The therapist will deliver evaluations, treatment plans, and functional training to facilitate patient recovery and independence.

Responsibilities:

• Assess patient needs through comprehensive evaluations and develop individualized treatment plans.
• Implement therapeutic interventions such as gait training, therapeutic exercises, and functional training.
• Document patient progress and treatment outcomes accurately using Meditech and Expanse systems.
• Collaborate with healthcare team members to coordinate care and optimize patient recovery.
• Educate patients and families on therapy goals, safety, and post-discharge care.
• Adhere to safety protocols and maintain compliance with all applicable certifications and standards.

Qualifications:

• Valid state license as a Physical Therapist.
• BLS certification required.
• Minimum of 2 years of clinical experience, preferably in SNF or rehab settings.
• Proven ability to work independently in a fast-paced environment.
• Experience with Meditech or Expanse documentation systems preferred.
• Strong interpersonal skills and adaptability to new hospital setups.
