Job Summary:
Provide specialized physical therapy services to patients in their home environment, promoting recovery and independence. Must be proficient in using Epic electronic medical records system to document care accurately.

Responsibilities:

• Assess patients' physical conditions and develop personalized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and functional abilities.
• Document patient progress and treatment details using Epic system.
• Educate patients and caregivers on exercises and injury prevention strategies.
• Collaborate with healthcare team members to coordinate care plans.

Qualifications:

• Possess valid physical therapy licensure and credentials.
• Demonstrate experience in home care settings.
• Proficient in using Epic electronic health record system.
• Display strong interpersonal and communication skills.
• Ability to adapt treatment approaches to individual patient needs.
