Job Summary:

Seeking a licensed Physical Therapist to provide skilled home health care services. The therapist will evaluate, plan, and implement treatment programs for adult patients in their homes, ensuring quality patient outcomes.

Responsibilities:

• Evaluate patients' functional status and develop personalized care plans.
• Administer therapeutic interventions according to physician orders.
• Document patient progress and treatment activities accurately within the required timeframe.
• Coordinate with healthcare team members to ensure continuity of care.
• Travel to patient homes within designated territory, up to 75 miles from the clinic.
• Maintain current licensure, CPR, and BLS certifications and adhere to all compliance standards.

Qualifications:

• Must have a minimum of 1 year of recent adult home health physical therapy experience.
• Possess a valid state license as a licensed Physical Therapist.
• Proficient in the use of EMR systems such as HomeCare HomeBase, Oasis, and Outlook.
• Demonstrate excellent clinical and documentation skills.
• Ability to travel and manage a flexible schedule, including potential weekend work.
• Reliable transportation with valid driver’s license and liability insurance.
