Job Summary:

The Physical Therapy Assistant (PTA) in Home Health provides skilled therapy services to adult patients in their homes, supporting recovery and mobility. The role involves travel within designated territories and collaboration with healthcare teams to deliver patient-centered care.

Responsibilities:

• Deliver therapeutic interventions to patients in their homes based on physician orders and treatment plans.
• Assess patient progress and modify therapy plans to optimize outcomes.
• Document clinical activities, observations, and patient responses accurately and timely.
• Coordinate with supervising therapists and healthcare providers regarding patient care and treatment adjustments.
• Travel to multiple patient locations within a 75-mile radius, managing a caseload of 5-6 patients per day.
• Comply with all documentation, safety, and compliance standards required by regulatory agencies and the organization.

Qualifications:

• Hold an active PTA license with proof of current licensure in the state.
• Minimum of 1 year of recent adult home health therapy experience.
• Proficient in documentation software including HomeCare HomeBase or comparable EMR systems.
• Demonstrate reliable transportation, valid driver’s license, and agency-required liability insurance.
• Ability to travel up to 75 miles from the home clinic, adjusting to flexible schedules.
• Strong interpersonal and communication skills to engage effectively with patients and team members.
