Job Summary:
Providing specialized nursing care for patients with nephrology conditions, the RN will assess patient needs, deliver effective treatment, and support overall kidney health management. This role emphasizes collaborating with healthcare teams to ensure optimal patient outcomes.

Responsibilities:

• Assess and monitor patients with nephrology conditions to identify care needs.
• Develop, implement, and evaluate individualized care plans in coordination with interdisciplinary teams.
• Educate patients and their caregivers on disease management, treatment options, and lifestyle modifications.
• Communicate clearly and professionally with patients, families, and healthcare colleagues.
• Prioritize patient safety by recognizing and escalating potential hazards or care gaps.
• Document nursing activities accurately using institutional information systems to facilitate quality and performance improvements.

Qualifications:

• Valid NYS RN license.
• Minimum of two (2) years of nephrology nursing experience.
• One (1) year of traveler nursing experience preferred.
• Current BLS certification.
• Epic electronic health record experience preferred.
• Demonstrated experience in patient assessment and critical thinking within nephrology.
