Job Summary:
Provides specialized speech and language therapy services to patients in home care or clinical settings. Conducts assessments, develops treatment plans, and delivers therapeutic interventions to improve communication, swallowing, and cognitive functions.

Responsibilities:

• Perform comprehensive patient assessments to determine appropriateness for therapy and establish baseline capabilities.
• Collaborate with patients, families, and healthcare team members to create and adjust individualized care plans.
• Implement evidence-based treatment programs to enhance communication, safe swallowing, and cognitive abilities.
• Educate patients and families on therapy techniques and self-management strategies using age-appropriate methods.
• Maintain accurate documentation of assessments, treatment sessions, progress, and discharge plans in compliance with regulatory and agency policies.
• Participate in interdisciplinary team meetings, provide discharge planning, and refer patients to additional resources as needed.

Qualifications:

• Hold a valid current NJ Speech-Language Pathologist license.
• Possess a Master’s degree from an accredited speech and language pathology program.
• Have at least one (1) year of experience in acute, subacute, or home care settings.
• Demonstrate strong communication and customer service skills.
• Show ability to work independently and travel to patient homes or facilities.
