Job Summary:

Seeking a dedicated Physical Therapist to provide skilled home health care through patient visits and assessments. The therapist will work independently, managing a caseload while adhering to industry standards and ensuring quality patient outcomes.

Responsibilities:

• Conduct initial evaluations, follow-up visits, and discharge planning for adult patients in a home health setting.
• Develop and implement personalized treatment plans based on patient assessments and clinical protocols.
• Document patient progress, revising care plans as needed, and complete all required documentation within 24 hours.
• Travel to patient homes, covering up to 75 miles as required, ensuring timely and efficient visits.
• Collaborate with interdisciplinary team members to coordinate care and optimize patient recovery.
• Adhere to compliance standards, including documentation, safety protocols, and insurance requirements.

Qualifications:

• Hold a valid state license as a Physical Therapist in good standing.
• Possess a minimum of 1 year of recent adult home health therapy experience.
• Proven proficiency in OASIS documentation and experience with EMR systems, specifically Home Care Home Base.
• Must have reliable transportation, valid driver’s license, and current liability insurance.
• Demonstrate excellent communication and time management skills.
• Be willing to travel within a 75-mile radius and accept frequent patient visits.
