Job Summary:
Provide professional physical therapy services to patients in a skilled nursing facility, ensuring the delivery of high-quality care aligned with each patient's needs. Responsible for evaluating, developing, and implementing treatment plans to promote patient recovery and mobility.

Responsibilities:

• Assess patient conditions and develop individualized treatment plans based on clinical best practices.
• Administer manual therapy, therapeutic exercises, and modalities to improve patient function.
• Document patient progress and adjust treatment strategies accordingly.
• Collaborate with healthcare team members to optimize patient outcomes.
• Comply with all regulatory standards and facility policies related to patient care.

Qualifications:

• Hold an active California physical therapy license.
• Possess at least 1 year of experience working in a skilled nursing facility setting.
• Demonstrate proficiency with NetHealth (Rehab Optima) or similar electronic health record systems.
• Exhibit strong clinical skills with the ability to evaluate and treat diverse patient conditions.
• Maintain current CPR and BLS certifications.
