Job Summary:
Provide compassionate nursing care to patients in a skilled nursing facility setting. Ensure adherence to all regulatory standards while delivering high-quality patient services.

Responsibilities:

• Administer prescribed medications and treatments accurately.
• Monitor and assess patient health statuses regularly.
• Document patient care and progress in medical records.
• Collaborate with interdisciplinary teams to develop care plans.
• Educate patients and families on health management and procedures.
• Comply with all facility policies, state licensing requirements, and safety protocols.

Qualifications:

• Hold a valid NYS Registered Nurse license.
• Possess a minimum of 2 years of experience in an SNF or LTC setting.
• Maintain current BLS certification.
• Demonstrate strong clinical assessment and communication skills.
• Ability to work independently and as part of a team.
