Job Summary:
Provides specialized speech therapy services to patients in a home care setting. Responsible for conducting assessments, developing treatment plans, and implementing interventions to improve communication and swallowing abilities.

Responsibilities:

• Perform initial and ongoing patient evaluations to determine speech and language needs.
• Develop individualized care plans aligned with patient goals and needs.
• Teach patients techniques to improve speech, voice, and language skills.
• Instruct patients on strengthening muscles and using compensatory strategies for safe swallowing.
• Maintain accurate records of assessments, treatment progress, and patient outcomes.
• Act as case manager for speech therapy cases, coordinating care and ensuring treatment compliance.

Qualifications:

• Current Maryland license as a Speech Language Pathologist.
• Prior home care experience preferred.
• CPR certification, valid driver’s license, and auto insurance required.
• Ability to work part-time, ideally three days per week (e.g., MWF or MTuTh).
• Capable of working 4 or 8-hour shifts with productivity goals of 3 or 6 points per day.
