Job Summary:

The Registered Nurse (RN) Home Health provider coordinates and delivers comprehensive nursing care to patients in their homes, following physician’s orders and agency protocols. Responsible for assessing patient needs, creating care plans, and supervising home health aides and practical nurses to ensure quality patient outcomes.

Responsibilities:

• Assess patient conditions and develop personalized care plans in accordance with physician directives.
• Provide skilled nursing services, including wound care, medication management, and health education.
• Supervise and evaluate home health aides and licensed practical/vocational nurses to maintain care standards.
• Document patient visits, treatments, and progress within the required EMR systems accurately and timely.

Qualifications:

• Hold an active RN license in good standing in the state of practice.
• Possess a minimum of 1 year of recent adult home health or hospice experience.
• Proficient in using HomeCare HomeBase (HCHB) EMR system, with Oasis proficiency preferred.
• Reliable transportation, a current driver’s license, and agency-required auto liability insurance.
• Ability to travel up to 75 miles from the assigned care location as needed.
