Job Summary:
Provide high-quality physical therapy services to residents, promoting mobility, strength, and overall well-being. Collaborate with healthcare teams to develop and implement personalized treatment plans to improve patient outcomes.

Responsibilities:

• Assess residents’ physical conditions and develop individualized treatment plans.
• Implement therapeutic interventions to improve movement, strength, and functional ability.
• Document patient progress and adjust therapy plans accordingly.
• Educate residents and staff on proper techniques to prevent injuries and enhance recovery.
• Coordinate with multidisciplinary teams to ensure comprehensive patient care.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience in outpatient or skilled nursing facilities.
• Strong assessment and clinical decision-making skills.
• Excellent communication and patient education abilities.
