Job Summary:
Seeking an experienced Registered Nurse for assessments in a home health setting. The role involves conducting comprehensive evaluations, creating individualized care plans, and ensuring seamless communication among care teams and consumers.

Responsibilities:

• Perform initial in-home health assessments to evaluate patient status, needs, strengths, and preferences.
• Review medical records and referral documentation to inform evaluations.
• Document and transmit evaluation data accurately into electronic systems.
• Communicate effectively with consumers regarding health care options and supports.
• Collaborate with interdisciplinary care teams to enhance care coordination.
• Maintain familiarity with community resources, payor guidelines, and network services.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years recent experience conducting comprehensive assessments in a home setting within the past fifteen years.
• Proficiency with electronic evaluation tools and technology.
• Strong organizational, interpersonal, and communication skills.
• Ability to work independently in a fast-paced, deadline-driven environment.
