Job Summary:

Provides comprehensive home nursing care to patients based on physician orders, ensuring quality and compliance through the nursing process. Supervises and evaluates home health aides and LPNs/LVNs while coordinating care delivery across assigned cases.

Responsibilities:

• Administer and coordinate patient care visits in accordance with physician directives and agency policies.
• Supervise and evaluate the performance of home health aides and LPNs/LVNs.
• Document patient progress and treatment plans accurately within required timeframes.
• Travel up to 75 miles to conduct home visits, including initial assessments, wound care, and follow-ups.
• Participate in periodic meetings and ensure compliance with all clinical and safety standards.
• Maintain proficiency with HomeCare HomeBase (HCHB) EMR system and Oasis documentation, completing all documentation timely.

Qualifications:

• Successful completion of licensed practical nursing (LPN) education program and valid state licensure.
• Minimum of one year of recent adult home health or hospice experience.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Proficiency with HomeCare HomeBase (HCHB) EMR system; Oasis experience preferred.
• Ability to travel within a 75-mile radius of the assigned care center.
• Strong communication, clinical, and documentation skills.
