Job Summary:

Provide psychosocial assessment, support, and counseling services to clients to promote emotional well-being and improve their overall quality of life. Collaborate with multidisciplinary teams to develop and implement individualized care plans.

Responsibilities:

• Conduct thorough client assessments to identify mental health and social service needs.
• Develop and implement personalized treatment plans in coordination with clients and healthcare providers.
• Provide individual, group, and family counseling sessions to address emotional and behavioral issues.
• Advocate for clients to access community resources, housing, and other essential services.
• Document client progress, treatment interventions, and service plans accurately and in accordance with regulations.
• Participate in team meetings and case consultations to ensure comprehensive client care.

Qualifications:

• Hold a valid state license or certification as a Licensed Clinical Social Worker (LCSW) or equivalent.
• Demonstrate experience in mental health, social services, or related fields.
• Possess strong interpersonal and communication skills.
• Exhibit ability to handle sensitive situations with empathy and professionalism.
• Maintain knowledge of community resources and relevant federal/state regulations.
