Job Summary:
Provides comprehensive occupational therapy services to patients with cognitive and activities of daily living (ADL) functioning disorders. Develops personalized care plans, administers therapeutic interventions, and evaluates progress to improve patient outcomes within regulatory and clinical standards.

Responsibilities:

• Screen and evaluate patients to identify cognitive and ADL impairments.
• Develop and implement evidence-based treatment plans aligned with regulatory requirements.
• Administer rehabilitative therapies including ADL retraining, therapeutic activities, exercises, modalities, and assistive device use.
• Assess patient progress, adjust treatments, and document outcomes accurately in clinical records.
• Communicate effectively with patients, families, caregivers, and interdisciplinary team members about treatment plans and progress.
• Ensure comprehensive discharge summaries and maintain all documentation per clinical guidelines.

Qualifications:

• Hold an active OT license with current state registration.
• Possess 1+ years of occupational therapy experience, with LTC or rehab background preferred.
• Demonstrate proficiency with standardized assessments and clinical documentation.
• Have a valid skills checklist within 1 year of submission.
• Be able to supervise OT assistants, aides, or students as required.
• New graduates with Level II experience considered.
