Job Description

Job Summary:

Assess, diagnose, and treat patients with communication, cognitive, or swallowing disorders, developing individualized plans of care that comply with regulatory standards. Collaborate with patients, families, and healthcare teams to optimize communication and functional outcomes.

Responsibilities:

  • Screen, evaluate, and rehabilitate patients with speech, language, cognitive, or swallowing impairments.
  • Utilize standardized assessments and evidence-based practices to guide clinical interventions.
  • Monitor patient progress and modify treatment plans accordingly, documenting all changes in accordance with regulations.
  • Provide education, counseling, and support to patients and their families regarding speech and swallowing issues.
  • Communicate effectively with supervisors and multidisciplinary team members on patient progress and treatment adjustments.
  • Maintain comprehensive records, including discharge summaries, treatment minutes, and regulatory documentation in electronic systems.

Qualifications:

  • Hold active ASHA certification as a Speech-Language Pathologist.
  • Possess a valid Oklahoma State License in Speech-Language Pathology.
  • Require at least one year of clinical experience, with LTC or rehab background preferred.
  • Be a CCC-certified SLP, with new graduates considered if relevant Level II experience.
  • Demonstrate adherence to ASHA and state practice acts, ethics, and confidentiality standards.
