Job Description
Job Summary:
Provides licensed practical nursing care to residents in a skilled nursing or long-term care setting under the supervision of the RN Supervisor. Ensures residents receive high-quality, compassionate care while maintaining regulatory compliance and fostering a safe, supportive environment.
Responsibilities:
- Administer and document medications, treatments, and monitoring procedures such as glucose checks as ordered.
- Accurately transcribe physician’s orders and update care plans to reflect residents’ current conditions.
- Review resident care with nursing assistants and supervise daily activities to ensure adherence to care plans.
- Communicate changes in resident status to the RN Supervisor and coordinate transfers with family and department heads.
- Collect laboratory specimens, prepare requisitions, and document procedures in the Electronic Health Record (EHR/EMR).
- Assist with resident admissions, discharges, and postmortem procedures, including collecting belongings and notifying relevant parties.
Qualifications:
- Valid LPN/LVN license in Pennsylvania and current CPR/BLS certification.
- Excellent clinical skills with knowledge of medication administration, resident care protocols, and safety procedures.
- Strong communication and documentation skills, with ability to work independently and within a team.
- Prior experience in LTC/SNF is preferred; new grads are welcome to apply.
- Ability to pass employment physicals and maintain good physical health, with knowledge of body mechanics.
