Job Summary:

The Registered Nurse in this role is responsible for conducting comprehensive assessments of patients within a home health setting, utilizing UAS criteria for enrollment and care planning. The position involves a mix of in-field and telehealth evaluations, ensuring high-quality, patient-centered care while adhering to regulatory standards.

Responsibilities:

• Perform initial in-home assessments evaluating patients' health status, strengths, care needs, and preferences to develop individualized long-term care plans.
• Review and interpret medical documentation and referral forms pertinent to each case.
• Enter evaluation data accurately into electronic systems and transmit as required.
• Document concerns, conflicting information, or issues encountered during assessments to ensure comprehensive records.
• Collaborate with consumers, healthcare teams, and service vendors to coordinate care options and support services.
• Communicate effectively with interdisciplinary teams to improve care coordination and delivery.
• Collect quality review data and record case notes into CRM systems to support outcome measurement.
• Identify opportunities for health promotion and disease prevention within the patient population.

Qualifications:

• Hold current RN licensure in New York State.
• Possess a minimum of two (2) years of recent home health assessment experience within the last ten (10) years.
• Demonstrate experience conducting assessments similar to UAS or OASIS, including work with CHHA, LHCSA, or MLTC providers.
• Have prior home care experience caring for a geriatric population and managing chronic diseases.
• Be proficient with electronic documentation systems and technology used in healthcare assessments.
• Preferably have recent experience conducting UAS-NY assessments, especially within the past 12 months.
