Job Summary:

Provides comprehensive home health nursing assessments and care planning for clients in their residences. Responsible for evaluating health status, coordinating services, and ensuring quality patient care in a fast-paced, deadline-driven environment.

Responsibilities:

• Perform in-home pre-evaluations to assess patient health status, needs, and preferences to develop individualized care plans.
• Review medical documentation and health referral forms to support accurate assessments.
• Document evaluation findings, concerns, and any conflicting information within electronic systems.
• Communicate care options, support services, and provider choices effectively with consumers.
• Collaborate with interdisciplinary teams to coordinate services and improve care delivery.
• Facilitate enrollment into managed care plans and maintain compliance with state and agency regulations.

Qualifications:

• Valid RN licensure in New York State.
• Minimum of two years recent experience conducting home health assessments within the last 15 years.
• Demonstrated proficiency with health assessment technology and documentation systems.
• Strong organizational, interpersonal, and communication skills.
• Ability to work independently in a fast-paced environment and manage complex tasks.
• Knowledge of state laws, regulations, and community resources related to home health nursing.
