Job Summary:
Provides specialized case management services to hospital patients, coordinating care plans and ensuring optimal health outcomes during inpatient stays. Collaborates with healthcare teams to deliver comprehensive patient care and facilitate efficient discharge planning.

Responsibilities:

• Assess patients’ clinical needs and develop individualized care management plans.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient services.
• Utilize Meditech, Midas, and Interqual tools to monitor patient progress and validate clinical criteria.
• Advocate for patients by ensuring timely interventions and discharge arrangements.
• Document all case management activities accurately within hospital systems.
• Comply with hospital policies and industry standards in patient care and case management procedures.

Qualifications:

• Possess a valid Registered Nurse (RN) license.
• Minimum of 2 years of acute (hospital) case management experience.
• Proficiency in Meditech, Midas, and Interqual software platforms.
• Strong understanding of hospital operations, patient flow, and clinical protocols.
• Demonstrate excellent communication and coordination skills.
• Ability to work flexible hours, including every other weekend.
