Job Summary:
Provides skilled physical therapy services to patients with physical functioning disorders and wound care needs. Develops and implements individualized treatment plans to improve patient outcomes and promote rehabilitation.

Responsibilities:

• Evaluate patients' physical conditions through standardized assessments and clinical observations.
• Develop and implement personalized treatment plans, utilizing manual techniques, therapeutic exercises, modalities, and supportive devices.
• Administer procedures and therapies according to best practices and regulatory standards.
• Monitor patient progress, adjust treatment plans, and document outcomes accurately in electronic health records.
• Communicate effectively with patients, families, caregivers, and healthcare team members regarding treatment progress and recommendations.
• Supervise and support physical therapist assistants, rehabilitation aides, and students as required.

Qualifications:

• Possess a valid Missouri State License to practice physical therapy.
• Have at least 1 year of physical therapy experience, preferably in LTC or rehab settings.
• Meet the minimum clinical experience requirement; new grads with Level II experience are eligible.
• Demonstrate knowledge of regulatory guidelines, clinical standards, and documentation protocols.
• Exhibit strong communication and team collaboration skills.
