Job Description
Job Summary:
Seeking a dedicated Speech Language Pathologist to assess, diagnose, and treat communication and swallowing disorders in a skilled nursing facility setting. The therapist will provide comprehensive care to patients across a skilled nursing unit on a full-time basis.
Responsibilities:
- Evaluate patients' speech, language, and swallowing functions to determine appropriate treatment plans.
- Develop and implement individualized therapy programs to improve communication skills and swallowing safety.
- Collaborate with interdisciplinary teams to coordinate patient care and progress.
- Document assessment findings, treatment goals, and progress notes accurately and timely.
- Educate patients and families on therapeutic exercises and strategies to support ongoing communication enhancement.
- Maintain compliance with regulatory standards and facility policies.
Qualifications:
- Valid state license or certification as a Speech Language Pathologist.
- Proven experience working in a skilled nursing or long-term care setting.
- Strong assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively in a team environment.
- Commitment to patient-centered care.
