Job Details

Job Summary:
Provide high-quality nursing care to residents in long-term care or skilled nursing facilities. Collaborate with healthcare teams to ensure residents' comfort, safety, and well-being in a contracted, fast-paced environment.

Responsibilities:

• Administer patient medications and treatments accurately according to established protocols.
• Monitor and record vital signs and patient conditions regularly.
• Assist residents with activities of daily living, including hygiene, mobility, and nutrition.
• Document all care provided in compliance with facility and legal requirements.
• Communicate effectively with patients, families, and healthcare team members regarding care plans and progress.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Must be PCC trained prior to assignment.
• Ability to demonstrate competency in long-term care and skilled nursing procedures.
• Provide a valid state or federal issued ID and wear a photo badge during shifts.
• Complete and provide documentation of COVID-19 vaccination or a religious/medical exemption.
