Job Summary:
Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Support patient health through clinical assessments, medication administration, and personalized care plans to ensure resident well-being.

Responsibilities:

• Administer medications and treatments accurately according to physicians' orders.
• Conduct patient assessments, monitor vital signs, and document clinical observations.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Ensure compliance with facility policies and state/federal regulations.
• Maintain patient safety and comfort by adhering to infection control protocols.
• Document all care activities thoroughly and promptly.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of recent experience in LTC/SNF settings.
• Ability to work quickly and accurately in a fast-paced environment.
• Strong communication and interpersonal skills.
• Knowledge of health and safety regulations applicable to long-term care.
• Must provide a valid ID and vaccination records or applicable exemptions.
