Job Summary:
Provide patient care and assistance in a long-term care or skilled nursing facility setting. Support healthcare teams in maintaining patient comfort and safety while adhering to facility policies and regulations.

Responsibilities:

• Assist patients with activities of daily living such as bathing, dressing, and feeding.
• Monitor and record patient vital signs and report any changes to the nursing staff.
• Follow infection control protocols, including proper use of PPE and hygiene procedures.
• Maintain a clean and safe environment for patients and staff.
• Document patient care activities accurately and timely.

Qualifications:

• Valid CNA certification in the state or federal registry.
• Reliable transportation to and from the facility.
• Proven experience in LTC/SNF or similar healthcare settings.
• Ability to work independently and as part of a team.
• Ability to pass a background check and health screenings.
