Job Details

Job Summary:
Provide skilled nursing care in a long-term care or skilled nursing facility setting. Ensure the well-being and safety of residents through clinical assessments and treatment planning.

Responsibilities:

• Administer medications and treatments according to physician orders and facility protocols.
• Monitor residents’ health status, vital signs, and progress, reporting any concerns promptly.
• Develop and update individualized care plans tailored to each resident’s needs.
• Collaborate with multidisciplinary team members to optimize patient outcomes.
• Document all clinical activities accurately in compliance with regulatory standards.
• Provide patient education and support to residents and their families.

Qualifications:

• Hold an active RN or LPN license valid in the state.
• Possess at least one year of experience in a MedSurg or LTC setting.
• Demonstrate strong assessment and critical thinking skills.
• Maintain current CPR and BLS certifications.
• Ability to work independently and as part of a team in a fast-paced environment.
